Upon activation of our service, we will do our best to ensure a timely delivery of every service requested. You will be contacted within 10 business days to verify that either the information has been completed/received, or to gather additional information as required.

Accuracy of the provided information is your responsibility, and Managed Carrier Services does not independently verify this information. If additional information cannot be gathered, the submission will be processed as received. You will be emailed a confirmation once our services have been rendered. Any changes/updates or additional information needed for the DOT Number will be communicated via email  within 10 business day after the submission has been filed.

By submitting this form, you verify that all the information provided is accurate and up-to-date to the best of your knowledge. Managed Carrier Services LLC  (“The Processor”) is not liable for any errors or inaccuracies in the application. The Processor will forward the provided information to the U.S. Department of Transportation (DOT)/ FMCSA or New York Department of Taxation as submitted. While efforts will be made to identify discrepancies, the Processor may reach out for verification if necessary.

Submission of a Report implies your authorization to act on behalf of the Subject of the Report.

While Managed Carrier Services LLC assists in report preparation and submission, acceptance by USDOT/FMCSA/FINCEN or compliance with 49 CFR/ the Corporate Transparency Act is not guaranteed. Managed Carrier Services LLC does not offer legal or professional advice.

CANCELLATION POLICY: 

Notice of Cancellation

You may cancel this transaction for refund within 48 hours of purchase date minus -2.9% processing fees and $.30 cents per transaction.

Refund Policy
Once an application is processed, submissions are final, and refunds will not be issued. Refund requests can only be made within 48 hours of filing, provided the application has not yet been processed or submitted to the DOT/FMCSA. Please note that credit card processing fees are non-refundable after submission.
